| 目的：评价射频针刀联合止痛利湿复方药膏对腰椎间盘突出症(lumbar disc herniation, LDH)寒湿痹阻证型患者的治疗效果。方法：收集2022年06月至2023年06月于河南省中医院收入住院的LDH患者96例,按随机数字表法排列2分组,各48例,对照组行射频针刀治疗,观察组采用射频针刀联合止痛利湿复方药膏治疗。2组治疗时长均为1个月,随访6个月。记录治疗前后及随访6个月时2组VAS、ODI评分与3项血清因子［前列腺素E2(PGE2)、肿瘤坏死因子-α(TNF-α)、白介素-1(IL- 1β)］并评估疗效。结果：治疗后2组患者VAS、ODI评分及3项血清因子指标与治疗前相比,均显著改善(P＜0.05),且观察组与对照组相比,下降更明显(P＜0.05)；随访结果显示,观察组VAS、ODI评分与对照组相比,更具稳定性(P＜0.05)。总有效率观察组(93.75%)与对照组(79.17%)相比具有差异(P＜0.05)。结论：射频针刀联合止痛利湿复方药膏对寒湿痹阻型腰椎间盘突出症的患者炎性因子具有抑制作用,减缓疼痛表达,改善腰椎功能,远期效果较稳定。 |

| Objective: To evaluate the therapeutic effect of radiofrequency needle knife combined with analgesic and diuretic compound ointment on patients with lumbar disc herniation (LDH) cold dampness obstruction syndrome. Method: 96 LDH patients admitted to Henan Provincial Hospital of Traditional Chinese Medicine from June 2022 to June 2023 were collected. They were randomly divided into two groups, with 48 patients in each group. The control group received radiofrequency needle knife treatment, while the observation group received radiofrequency needle knife combined with pain relieving and dampness reducing compound ointment treatment. Both groups had a treatment duration of 1 month and were followed up for 6 months. Record the VAS, ODI scores, and three serum factors (prostaglandin E2 (PGE2) and tumor necrosis factor) in two groups before and after treatment, as well as at 6 months of follow-up- α (TNF)- α)、 Interleukin-1 (IL-1) β)］ And evaluate the therapeutic effect. Result: After treatment, VAS, ODI scores, and three serum factor indicators in both groups of patients significantly improved compared to before treatment (P<0.05), and the observation group showed a more significant decrease compared to the control group (P<0.05); The follow-up results showed that the VAS and ODI scores in the observation group were more stable than those in the control group (P<0.05). There was a difference in total effective rate between the observation group (93.75%) and the control group (79.17%) (P<0.05). Conclusion: The combination of radiofrequency needle knife and analgesic and diuretic compound ointment has an inhibitory effect on inflammatory factors in patients with cold dampness obstruction type lumbar disc herniation, slows down pain expression, improves lumbar function, and has a relatively stable long-term effect. |
