| 目的 评价温阳活血利水方药对急性失代偿性心力衰竭（acute decompensated heart failure，ADHF）阳虚水泛证患者血流动力学的影响。方法 选取2022年3月—2024年5月本院收治的ADHF患者50例为研究对象，按照随机数字表法分为治疗组22例和对照组21例。对照组患者进行西医常规治疗，治疗组在对照组基础上加用温阳活血利水方药。比较2组患者治疗前后每搏输出量（stroke volume，SV）、每搏指数（stroke volume index，SVI）、心排血量（cardiac output，CO）、心脏指数（cardiac index，CI）、全身血管阻力（systemic vascular resistance，SVR）、全身血管阻力指数（systemic vascular resistance index，SVRI）、氧运送量（oxygen delivery，DO2）、峰值流速（velocity peak，VPK）血流动力学指标。结果 治疗后，2组患者SV、SVI、CO、CI、DO2、VPK水平均较治疗前显著上升（P＜0.05），且治疗组显著高于对照组（P＜0.05）；2组患者SVR、SVRI水平均较治疗前明显下降（P＜0.05），且治疗组显著低于对照组（P＜0.05）。结论 温阳活血利水方药能够显著增强ADHF患者心脏泵血功能，增加心输出量，降低外周血管阻力，改善周围组织器官的氧供。 |

| Objective Evaluate the impact of Wenyang Huoxue Lishui Decoction on hemodynamics in patients with yang deficiency and water retention syndrome of acute decompensated heart failure (ADHF).Methods Fifty patients with ADHF admitted to our hospital from March 2022 to May 2024 were selected as the study subjects. According to the random number table method, 22 patients were assigned to the treatment group and 21 patients to the control group.Patients in the control group received conventional Western medical treatment, while patients in the treatment group received additional treatment with Wenyang Huoxue Lishui Decoction on the basis of the control group's treatment.Compare the hemodynamic parameters of stroke volume (SV), stroke volume index (SVI), cardiac output (CO), cardiac index (CI), systemic vascular resistance (SVR), systemic vascular resistance index (SVRI), oxygen delivery (DO2), and velocity peak (VPK) in two groups of patients before and after treatment. Results After treatment, the levels of SV, SVI, CO, CI, DO2, and VPK in both groups of patients significantly increased compared to before treatment (P<0.05), and the treatment group showed significantly higher levels than the control group (P<0.05).The levels of SVR and SVRI in both groups of patients decreased significantly compared to before treatment (P<0.05), and those in the treatment group were significantly lower than those in the control group (P<0.05). Conclusion Wenyang Huoxue Lishui Decoction can significantly enhance the heart's blood-pumping function in patients with ADHF, increase cardiac output, reduce peripheral vascular resistance, and improve oxygen supply to surrounding tissues and organs. |
