目的 观察2型糖尿病合并认知功能障碍患者不同体质人群中胰岛素抵抗和血清 25-羟维生素D3(25(OH)D3)水平的差异,并探讨体质因素与胰岛素抵抗、25(OH)D3水平的相关性;方法 选取确诊的2型糖尿病合并认知功能障碍患者共 120 例,辨识出痰湿质组和平和质组,检测其空腹血糖(FBG)、空腹胰岛素(FINS)、餐后2小时血糖(PPG)、糖化血红蛋白(HbA1c)、总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白(LDL)、25 (OH) D3;结果 两组患者血脂、血糖、HbA1c检测值组间比较无统计学差异(P>0.05),痰湿质组患者的空腹胰岛素检测值高于平和质组,同时计算的HOME抵抗指数也高于平和质组,差异均具有统计学意义(P<0.05);25(OH)D3检测值低于平和质组,差异具有统计学意义(P<0.05);结论 在2型糖尿病合并认知功能障碍患者中,痰湿质组胰岛素抵抗更严重,25(OH)D3水平偏低,显示该人群中体质因素和胰岛素抵抗程度、25(OH)D3水平存在相关性。 |

Objective: To observe the differences in insulin resistance and serum 25-hydroxyvitamin D3 (25(OH)D3) levels among patients with type 2 diabetes mellitus (T2DM) and cognitive impairment who have different physical constitutions, and to explore the correlation between physical constitution and insulin resistance, as well as 25(OH)D3 levels.Methods: A total of 120 patients with diagnosed T2DM and cognitive impairment were selected and divided into a Phlegm-Damp constitution group and a Balanced constitution group. Fasting blood glucose (FBG), fasting insulin (FINS), postprandial 2-hour blood glucose (PPG), glycated hemoglobin (HbA1c), total cholesterol (TC), triglycerides (TG), low-density lipoprotein (LDL), and 25(OH)D3 were measured.Results: There were no statistically significant differences in blood lipids, blood glucose, and HbA1c levels between the two groups (P > 0.05). The FINS levels and calculated Homeostasis Model Assessment of Insulin Resistance (HOMA-IR) index were higher in the Phlegm-Damp constitution group compared to the Balanced constitution group, with statistically significant differences (P < 0.05). Additionally, the 25(OH)D3 levels were lower in the Phlegm-Damp constitution group, with a statistically significant difference (P < 0.05).Conclusion: In patients with T2DM and cognitive impairment, those with a Phlegm-Damp constitution exhibit more severe insulin resistance and lower 25(OH)D3 levels. This indicates a correlation between physical constitution and the degree of insulin resistance, as well as 25(OH)D3 levels in this population. |
