目的:观察复方清胰汤通过调节肠道菌群,增加双歧杆菌数量改善急性胰腺炎的炎症并促进微循环障碍恢复的疗效及机制。方法:选取2023年1月至2024年10月就诊于我院的AP患者70例,平均分为对照组和实验组,对照组采用常规西医治疗,实验组在常规西医治疗基础上加用口服复方清胰汤,分别在治疗前、治疗后第3天、治疗后第7天采血液样本送检,观察血常规、炎症因子、凝血常规、血脂水平、Lac、双歧杆菌数量的变化。结果:治疗前70例患者的各化验指标间均具有相关性,为强相关(P<0.01),其中WBC、NEUT、RDW、HCT、MPV、PDW、PT、APTT、DD、TC、TG、IL-6、CRP、Lac互相均为正相关,同双歧杆菌、PLT呈负相关,双歧杆菌、PLT互为正相关;治疗前两组患者WBC、NEUT、RDW、HCT、PLT、MPV、PDW、双歧杆菌、PT、APTT、DD、TC、TG、IL-6、CRP、Lac无统计学差异性(P>0.05),两组患者治疗后第3天、治疗后第7天同期WBC、NEUT、RDW、HCT、PLT、MPV、PDW、双歧杆菌、PT、APTT、DD、TC、TG、IL-6、CRP、Lac均存在统计学差异性(P<0.05),实验组患者的WBC、NEUT、RDW、HCT、PLT、MPV、PDW、双歧杆菌、PT、APTT、TC、TG、Lac、IL-6、CRP降低较对照组更显著(P<0.05),PLT、D-D上升较对照组更显著(P>0.05);经过治疗,患者双歧杆菌数量、PLT、D-D均呈上升趋势,WBC、NEUT、RDW、HCT、MPV、PDW、PT、APTT、TC、TG、LAC、IL-6、CRP均呈下降趋势。结论:复方清胰汤能够通过增加双歧杆菌数量,降低患者胰腺炎症水平,并能够促进微循环障碍的恢复,其机制可能与肠屏障和血管内皮损伤修复、血液流变学的恢复及微血栓的分解有关。 |

Objective: To observe the efficacy and mechanism of Compound Pancreatic Cleansing Soup in improving the inflammation and promoting the recovery of microcirculation disorder in acute pancreatitis by increasing the number of bifidobacteria. Methods: 70 cases of AP patients who visited our hospital from January 2023 to September 2024 were selected and equally divided into control group and experimental group, the control group was treated with conventional western medicine, and the experimental group added oral Fufang Qingpancreas Tang on the basis of the conventional western medicine, blood samples were taken and sent to us for examination before the treatment, the 3rd day after the treatment, and the 7th day after the treatment, respectively, and we observed the changes of blood routine, inflammatory factors, coagulation routine, blood lipid level, Lac, and bifidobacteria count changes. Results: Before treatment, 70 patients had correlation between each laboratory index, which was strong correlation (P < 0.01), among which WBC, NEUT, RDW, HCT, MPV, PDW, PT, APTT, DD, TC, TG, IL-6, CRP, and Lac were positively correlated with each other, negatively correlated with bifidobacteria and PLT, and positively correlated with bifidobacteria and PLT with each other; There was no statistically significant difference in WBC, NEUT, RDW, HCT, PLT, MPV, PDW, bifidobacteria, PT, APTT, DD, TC, TG, IL-6, CRP, Lac between the two groups before the treatment (P> 0.05), and in the same period of time on the 3rd day after the treatment and on the 7th day after the treatment in both groups, WBC, NEUT, RDW, HCT, PLT, MPV, PDW, bifidobacteria, PT, APTT, DD, TC, TG, IL-6, CRP, Lac were statistically different (P< 0.05), and the experimental group patients" WBC, NEUT, RDW, HCT, PLT, MPV, PDW, bifidobacteria, PT, APTT, TC, TG, Lac, IL-6, and CRP were reduced more than that in the control group (P< 0.05), and PLT, D-D increased more significantly than the control group (P> 0.05); after treatment, the patients" bifidobacteria counts, PLT, D-D showed an upward trend, and WBC, NEUT, RDW, HCT, MPV, PDW, PT, APTT, TC, TG, Lac, IL-6, CRP showed a downward trend. Conclusion: Compound Pancreatic Cleansing Soup was able to reduce the level of pancreatic inflammation in patients by increasing the number of bifidobacteria and was able to promote the recovery of microcirculatory disorders, and its mechanism may be related to the repair of intestinal barrier and vascular endothelial damage, the recovery of blood rheology and the decomposition of microthrombus. |
