| 目的 通过临床观察和网络药理学探讨科室自拟方-扶正化瘀汤(FZHYD)治疗脓毒症的临床疗效和作用机制。方法 采用随机对照试验设计,将符合纳入标准的60例患者,随机分配至治疗组(常规治疗+扶正化瘀汤)和对照组(常规治疗),疗程7天。主要评价指标包括临床总有效率、炎症标志物(CRP、PCT、IL-6)及器官功能评分(SOFA、GCS)。利用网络药理学分析扶正化瘀汤治疗脓毒症的作用机制。结果 临床研究显示,治疗组总有效率显著高于对照组,且炎症标志物改善更显著。网络药理学筛选出槲皮素、山柰酚等12个核心成分,通过调控AKT1、IL6、CASP3等23个关键靶点,主要涉及PI3K-AKT信号通路、IL-17通路及AGE-RAGE通路等10条生物通路。结论 扶正化瘀汤通过多靶点、多通路改善脓毒症患者的炎症反应,提高临床有效率。 |

| ObjectiveTo explore the clinical efficacy and mechanism of action of Fuzheng Huayu Decoction (FZHYD), a self-prescribed formula from our department, in the treatment of sepsis through clinical observation and network pharmacology.MethodsA randomized controlled trial was conducted, enrolling 60 eligible patients who were randomly assigned to either the treatment group (conventional therapy + FZHYD) or the control group (conventional therapy alone) for a 7-day course. Primary outcome measures included overall clinical response rate, inflammatory markers (CRP, PCT, IL-6), and organ function scores (SOFA, GCS). Network pharmacology was employed to analyze the mechanism of FZHYD in sepsis treatment.ResultsClinical findings demonstrated that the treatment group exhibited a significantly higher overall response rate and more pronounced improvement in inflammatory markers compared to the control group. Network pharmacology identified 12 core active components (e.g., quercetin, kaempferol) and 23 key targets(e.g., AKT1, IL6, CASP3), primarily involving 10 biological pathways such as the PI3K-AKT signaling pathway, IL-17 pathway, and AGE-RAGE pathway.ConclusionFZHYD improves inflammatory responses and enhances clinical efficacy in sepsis patients through multi-target and multi-pathway mechanisms. |
