      目的:探究加味济生肾气汤对糖尿病肾病大鼠肾脏的抗纤维化作用及其作用机制。方法:将66只SD大鼠随机分成对照组、模型组、羟苯磺酸钙组、加味济低、中、高剂量组,每组11只。脲佐菌素(STZ)诱导构建糖尿病肾病大鼠模型,根据分组分别给药6周后,取大鼠肾脏,以HE染色的方法观察肾脏病理变化,以Masson染色的方法观察肾脏的纤维化程度;以ELISA的方法检测肾脏组织液中α平滑肌肌动蛋白(α-SMA)、母亲DPP同源物3(Smad3)、I型胶原(Col-I)、III型胶原(Col-III)、转化生长因子β(TGF-β)、血管内皮生长因子(VEGF)的水平。结果:与对照组相比,模型组肾小球体积增加,系膜细胞和系膜基质增生,系膜区变宽,基底膜增厚,毛细血管环增多;肾小管上皮细胞肿胀,融合,坏死;肾小球系膜区大量炎性细胞浸润。在羟苯磺酸钙组、加味济中剂量组和加味济高剂量组中上述症状得以缓解,且肾小管间质纤维化程度降低。与模型组比较,羟苯磺酸钙组、加味济高剂量组大鼠肾组织液中的α-SMA,Smad3,Col-III,Col-I,TGF-β,VEGF水平均显著降低(P<0.05)。结论:加味济生肾气汤可减缓糖尿病肾病大鼠的肾脏纤维化进程,其作用机制可能与调节TGF-β/Smad通路有关。

      Objective: To explore the anti-fibrosis effect and mechanism of Jiawei Jisheng Shenqi Decoction on kidney of diabetic nephropathy rats. Methods: 66 SD rats were randomly divided into control group, model group, calcium dobesilate group, modified Jisheng Shenqi Decoction low, middle and high density group. Each group had 11 rats. Diabetic nephropathy rats were induced by STZ. After 6 weeks of administration, the kidneys of rats were taken. The pathological changes of the kidneys were observed by HE staining and the degree of fibrosis was observed by Masson staining; The levels of α-smooth muscle actin (α-SMA), mother against decapentaplegic homolog 3 (Smad3), collagen type I (Col-I), Col-III, transforming growth factor-β (TGF-β), and vascular endothelial growth factor (VEGF) were measured by ELISA. Results: Compared with control group, in model group, glomerular volume increased, mesangial cells and mesangial matrix proliferated, mesangial area widened, basement membrane thickened, and capillary rings increased; Renal tubular epithelial cells were swollen, fused and necrotic; There were a large number of inflammatory cells in the mesangial area. The above symptoms were relieved in the calcium dobesilate group, the middle dose group and the high dose group of Jiaweij, and the degree of tubulointerstitial fibrosis was reduced. Compared with the model group, the levels of α-SMA, Smad3, Col-III, Col-I, TGF-β and VEGF in the kidney tissue fluid of rats in the calcium dobesilate group and the high dose group of Jiaweiji were significantly reduced (P<0.05). Conclusion: Jiawei Jisheng Shenqi Decoction can reduce the fibrosis of kidney in diabetic nephropathy rats by regulating the TGF-β/Smad pathways .
