中医认为,血瘀是“积聚”形成的必要条件,肿瘤亦常伴有血管的异常增生。肿瘤微环境中,“血管生成开关”持续开启,血管新生被大幅上调以满足肿瘤细胞对于氧气和营养的需求,这是肿瘤快速生长的关键点。然而,肿瘤新生血管具有口径不一、发育不良、血流紊乱、出血渗漏等特征,无法实际缓解缺氧。肿瘤异常血管新生的细胞学基础是内皮细胞的快速无序增殖和周细胞的覆盖不全,这与中医血瘀理论相契合。因此,肿瘤血管新生可能是肿瘤血瘀证候的微观体现。应用活血化瘀法促进肿瘤血管正常化,以改善肿瘤微环境的缺氧、酸中毒、免疫抑制和药物耐受,可能是治疗肿瘤的新思路。 |

Blood stasis is a necessary condition for the formation of "accumulation" in Traditional Chinese medicine. Correspondingly, tumors are often accompanied by abnormal proliferation of blood vessels. The "angiogenic switch continuously turned on in Tumor Microenvironment (TME) to meet the needs of tumor cells for oxygen and nutrients, which is the key point for the rapid growth of tumors. However, tumor neovasculature is characterized by different calibers, dysplasia, erratic blood flow and leakiness, which cannot actually ameliorate hypoxia. The cytological basis of abnormal tumor angiogenesis is the rapid and disordered proliferation of endothelial cells and the defective coverage of pericytes, which is consistent with the theory of blood stasis in traditional Chinese medicine. Therefore, tumor angiogenesis may be the microscopic features of tumor blood stasis syndrome. Facilitating the vessel normalization of tumor by promoting blood circulation and removing blood stasis contributes to ameliorate the hypoxia, acidosis, immunosuppression and drug tolerance, which provides a new idea for the treatment of tumors. |
